Heart to Heart Hospice is looking for compassionate volunteers to help support patients and families through weekly check-in calls.
As a Tuck-In Call Volunteer, you will make friendly phone calls to patients or their caregivers to ensure they feel supported and prepared for the days ahead. These calls help our care team confirm that patients are comfortable, have the supplies they need, and know they are not alone.
What You’ll Do
- Make short, supportive phone calls to patients or caregivers
- Check in about comfort, medications, supplies, and equipment
- Help ensure patients are prepared for the weekend through, Weekly Tuck-In Calls
- Make Post-Admission Check-In Calls to welcome new patients and families to hospice services
- Share any questions or concerns with the hospice care team for follow-up
